Overview
Surgery Monster is the Room 8 emergency event that triggers in post-operation recovery. A massive purple blob with same-colour tentacles drapes itself over the patient on the surgery table and starts a 45-second countdown. The patient is the victim, not you — Surgery Monster doesn't drain sanity or deal player damage. What it does is convert a recovering patient into a recorded death if the timer expires, which damages your shift score and your animal-survival rate.
The critical twist: the 45-second timer is persistent. If you start a new surgery during the event, the clock keeps running — it doesn't reset to 45. So queueing fresh patients into Room 8 while a Surgery Monster is active is just stacking pressure. The other critical detail is that reflexive enemy responses (Gun, Taser, Coffee gift) all auto-kill the patient instead of resolving the event. How to Handle
When the Surgery Monster spawns, abandon every other Room 8 task. Open the surgery tray and execute the sequence in order: Scissors, then Transplant, then Scissors again, then Transplant again. After that, the system randomises three treatments from the tray — you have to read each prompt and apply the indicated tool quickly. The total budget is 45 seconds for all 7 steps, so the practiced sequence really matters.
Do not improvise. Do not pull a Gun, do not reach for the Taser, do not try to give the monster Coffee. Each of those is a guaranteed patient kill — they're listed as 'do not do' in every documented walkthrough because new players reflexively try them. The encounter is designed as a procedural puzzle, not a combat threat.
If another patient is queued for Room 8 while the event is active, hold them at intake. Letting them onto the surgery table won't reset the 45-second clock; it just gives you a second timer to fail on top of the first one. Park the queue in the lobby until Surgery Monster resolves.
Multiplayer division of labour: one teammate runs the tool sequence at the table while another holds the door against incoming patients and against any wandering enemies (Skinwalker, Hiders) that might disrupt the operation.
