Overview

⚠️ Major spoilers for the Barney storyline follow. The Report Barney Ending is the ethically-clean outcome of the Barney encounter arc — and the mechanically-worse one. Per /wiki/npcs/barney, Barney is the serial killer behind the hospital's underlying plot. Reporting him to Officer Duckman is the canonically 'right' choice — but it triggers a mid-shift medical cascade where Duckman gets attacked and the player must save his life.

The arc shares the first four stages with the Hide ending (coffee, briefcase, photograph, scalpel). The divergence happens at stage 5: when Barney asks to be hidden behind the front desk, the player refuses. Officer Duckman arrives, finds the suspect, calls for arrest assistance — and gets attacked by Barney before the arrest can complete. Duckman is incapacitated.

Per /wiki/npcs/officer-duckman: the player must then apply standard medical treatment to keep Duckman alive. He requires this treatment to survive. If you fail to treat him in time, Duckman dies — adding a failure cascade on top of an already-eventful shift. If you succeed, Duckman survives but per Fandom 'displays minimal gratitude even when rescued.'

How to Achieve This Ending

The Report Barney Ending requires the same active engagement with Barney's staged interaction sequence as the Hide ending — but with the opposite choice at stage 5.

Stage 1 — Coffee request: bring a Coffee cup to Barney when he first appears (see /wiki/items/coffee). Same as the Hide path.

Stage 2 — Red briefcase safeguard: accept the briefcase. Same as Hide.

Stage 3 — Photograph: take a photograph of Barney with the Photo Camera. Per /wiki/npcs/officer-duckman, this photograph somehow ends up in Duckman's hands as evidence — the mechanism isn't explained in-fiction. Critical: this photograph is what enables Duckman's investigation regardless of which ending you pick.

Stage 4 — Scalpel: provide the scalpel when demanded. Same as Hide path. (The scalpel later appears as a knife in Barney's anomalous-form reveal during the attack on Duckman.)

Stage 5 — REFUSE the hide request: when Officer Duckman approaches and Barney asks to be hidden, decline. This is the critical divergence from the Hide ending.

What happens next: Officer Duckman arrives at the front desk. He calls for arrest assistance upon finding Barney (who has not been hidden). Barney transforms into his anomalous form — sharp-toothed grin, hollow black eyes with white dot centres, knife in hand — and attacks Duckman. Duckman is incapacitated and falls to the floor.

Mid-shift cascade: at this point, Officer Duckman is effectively a critical-treatment patient. Apply standard medical treatment to keep him alive — per AH content, the treatment requirement is comparable to the Critical Patient event (100-120 second window). Use Bandages for visible wounds, Smelling Salts to wake him post-treatment. If you don't have these items on hand, Officer Duckman dies.
